Request for Notarization of Transcript / Diploma

This form is for current and former graduate students to request notarization of transcripts and/or diplomas.

Please allow up to 2 weeks for processing.

Notarized transcript
When ordering notarized copies of your official transcript, you must also order copies of your transcript through Parchment online ordering service. You should order as many copies of your transcript that you need notarized.

To enable us to print your transcript on-site, please select Local Pickup as the transcript delivery method.


Notarized diploma
When ordering notarized copies of your original diploma, you must send or bring in the original diploma, as we do not maintain copies. If you have lost your original diploma or need a replacement please order a replacement diploma through Parchment online ordering service


Replacement diploma ordering options
You have two options:
  1. Request that the diploma be mailed to your home address and bring it to the Graduate Registrar's Office for notarization - OR - 

  2. If you are unable to bring it to the Graduate Registrar's Office you can request that the diploma be mailed directly to us at:
